UCAN Back to School Supplies Event

UCAN Annual Back to School Supplies Event August 10

The Urbandale Community Action Network (UCAN) has multiple programs which can assist with school supplies, clothing, shoes, and more for your family. All families are required to register with UCAN once a year. The UCAN Annual Back to School event will be held from 3:00 p.m. – 7:00 p.m. on Wednesday, August 10, 2022 at the UCAN Office located at Aldersgate Church (lower level) 3600 75th Street, Urbandale. 

Pre-registration for the Back to School Supplies event is not required. However, walk-in registration will only be accepted starting at 6:00 p.m. on August 10 and will be based on availability. Walk-in registration still requires the following: 1) identification for everyone in your household (e.g. driver’s license, passport, school ID, medical card, birth certificate, or correspondence from school that includes your children’s names), 2) proof of residency (e.g. utility bill, lease agreement, mortgage statement), and 3) income verification (e.g. two week pay stub or income tax return from last year).

If time permits, please complete the following two forms: 

  • 2022 UCAN Annual Client Application Please fill out the Client Application form and include all supporting documentation. You’re welcome to take photos of your supporting documentation (proof of residence, income, etc.) and upload those photos in the application. All information requested must be provided in order to be eligible for assistance. If you have not completed a Client Application with UCAN before July 31, 2021, please contact UCAN at 515-334-2684 or to schedule an appointment.
  • Back to School 2022 Once you have completed the Client Application, please register for the Back to School 2022 event (click ‘Register Here’) on the link provided for Back to School 2022.
