Kindergarten Registration 2021-2022

To be eligible for kindergarten a child must be 5 years old by September 15, 2021.

The Urbandale Community School District (Urbandale CSD) welcomes your kindergarten student! We take great care in creating a culture of learning in our classrooms to help make the transition to kindergarten a positive experience for your child and family. Our focus is to help each child learn, grow, and achieve success on the pathway of lifelong learning.

Kindergarten students must be residents of the Urbandale CSD or file the appropriate paperwork. Please see the section of our website about Open Enrollment if you do not live within district boundaries. Please Note: The Traditional Calendar is followed by all schools except Rolling Green. Register your child for kindergarten!


Please complete this short form to request the registration materials in digital format. We’d like all pre-registration packets submitted by early March.

Please submit the additional registration documents listed on the cover sheet via email to or via fax at 515-457-5018. If you have questions or need assistance, please contact Sandy Walters at the previously listed email or call 515-457-5011.


Once all required documents have been received, the Registrar will enter all preliminary kindergarten registration information into our online registration system. That process will be completed by the end of April/early May. Shortly thereafter, you will receive an email and/or letter that contains instructions and a “SnapCode” for your kindergarten student. With your “SnapCode” handy, please login to InfoSnap as directed and create an account to complete the online portion of registration for your kindergarten student. (If you have older siblings in the district, you may login with the account you have already created.) You will receive a confirmation at the end of the online program that your student’s registration has been completed. If you have email, you should also receive an email confirmation. (Note: If you do not receive a confirmation email, please contact Sandy Walters at or call 515-457-5011.) 


In order to help streamline the registration process for your child to start kindergarten, we’d like to request receiving these health forms by May 1 if possible. Please note that Iowa Law requires a parent to provide evidence of  required immunizations upon enrollment. If your child is missing any of the required immunizations, your child may be enrolled provisionally for 60 calendar days if she/he has received at least one dose of each required vaccine. After the 60 days, your child will be excluded from school until he/she receives the immunizations. Please bring the completed forms to your child’s school or the District Administration Office.

Our request when printing/submitting forms: please print 1-sided (each form on its own sheet of paper). Also, if you bring original documents, we will copy and return the originals to you. Thank you!

Bus Transportation
Learn more about Bus Transportation here. To be on a route for the first day of school, your request needs to be received no later than July 1st for Rolling Green and August 1st for all other schools.  We will be happy to add your student after the due date, but the bus stop locations will be set; thus, students may have to walk farther and possibly wait several days before they can begin riding the bus. Free Meals For All Students Throughout the 2021-22 School Year
Urbandale Community Schools elected to participate in the Seamless Summer Option (SSO) allowing us to provide a free breakfast and a free lunch to all students enrolled in UCSD throughout the 2021-2022 school year! You’re welcome to complete the online Iowa Eligibility Application included below (note: the Iowa Eligibility Application is also known as the Free and Reduced Meal Application). Only one Iowa Eligibility Application per household is required. Families are asked to complete the Iowa Eligibility Application form as your free or reduced status can impact fees for books & materials, driver’s education, bussing costs, and other district services. Please know, while meals (breakfast and lunch) are free this school year, any à la carte items such as milk, chips, cookies, etc. will still require payment to purchase.

Student Fee Waiver
If your child(ren) qualifies for free and reduced meals per the Iowa Eligibility Application, they may also be eligible for other benefits such as reduced fees for books, materials, driver’s education, bussing, and other district services. In order to receive these additional benefits, families need to apply for the student fee waiver using this form: 2021-2022 Application for Waiver of Confidentiality and School Fees

Parent Teacher Organization (PTO)
If you are a parent, guardian or teacher, you are automatically a member of the PTO. There is no application to complete and no dues to pay. The PTO holds monthly meetings throughout the school year. You should check with your school to find out when the meetings are, what time and where they are held.

Safety City: Hosted by the Urbandale Police Department—Dates Confirmed
Safety City is a week-long summer program for children entering kindergarten in the fall of 2021. Children will learn about traffic safety, fire safety, home, playground and other kinds of personal safety in an entertaining, positive environment. To make learning about safety fun, Safety City incorporates songs, story books, films, games, and artwork in addition to riding Big Wheels in a mock city setting. Safety City also provides the experience of riding on a school bus to the Urbandale Police and Fire Departments for tours. Safety City is taught by Urbandale Police Officers and staff who are assisted by a team of student helpers. Register for Safety City by May 2, 2021. Special Education, English Language Learners, Title 1
These programs are designed to provide an extra boost to the students who may benefit from the experience. Special Program teachers and the classroom teacher focus on the same objectives, helping students become even more successful at reading, writing, language and thinking. Students may be recommended for special programs by kindergarten staff after the school year begins. Please visit our Student Services page for more information.
